Neuroleptic malignant syndrome is a rare but potentially lethal, rare reaction to neuroleptics which is characterized by altered levels of consciousness, extrapyramidal effects, autonomic instability, hyperthermia, and elevated serum creatine phosphokinase levels. The most serious complication of neuroleptic malignant syndrome is acute renal failure. We investigated six cases of neuroleptic malignant syndrome associated with myoglobulinemic acute renal failure due to rhabdomyolysis and effect of hemodialysis or hemodiafiltration. The patients were five males and one female with a mean age of 43.5 yr. All of the patients, who developed acute renal failure induced from rhabdomyolysis, had previously received butyrophenone (haloperidol), phenothiazine, benzamide, iminomide, benzisoxazole, antidepressants, and hypnotics (benzodiazepine and barbiturate) for the treatment of schizophrenia. As a result, myoglobulinemic acute renal failure associated with neuroleptic malignant syndrome was successfully treated by hemodialysis or hemodiafiltration.

We evaluated our clinical experience with zonisamide, a broad-spectrum antiepileptic drug, in a group of children with predominantly medically refractory epilepsy. A retrospective chart review was conducted on patients at our tertiary referral center following Institutional Review Board approval. Observers documented reports of seizure frequency, and seizure types were identified either clinically or by prior video-electroencephalography monitoring. We identified 68 patients (age range 1.9-18.1 years [median 6.9 years]; male to female ratio 1.3:1) treated with zonisamide for 0.7 to 28.9 months; at the last visit, 22% and 78% were on monotherapy and adjunctive therapy, respectively. The median duration of treatment and maintenance dose at the end of the follow-up were 11.2 months and 8.0 mg/kg/day, respectively. Seizure types included generalized (primary generalized tonic-clonic, myoclonic, tonic, atonic, absence) and partial (simple, complex, and secondarily generalized tonic-clonic seizures); 10 (15%) patients had both partial and generalized seizures. Sixteen (25.8%) patients were seizure free, although five of them were already in remission prior to starting zonisamide. Thirteen (21.0%) patients had a >= 50% seizure reduction, 10 (16.1%) patients had a < 50% seizure reduction, 14 (22.6%) had no improvement in baseline seizures, and 9 (14.5%) reported having increased seizures. The latter were mostly associated with dosage alterations in concomitant antiepileptic drugs. Common side effects were central nervous system related, including behavioral or psychiatric (23.5%), cognitive dysfunction (12.0%), and sedation (10.3%). Eleven (16.2%) patients ultimately discontinued zonisamide, but only five were strictly due to side effects. Zonisamide is clinically effective against multiple seizure types in a significant proportion of children with epilepsy across a broad age range. Isomerization and decomposition of 1,2-benzisoxazole were studied behind reflected shock waves in a pressurized driver, single-pulse shock tube. It isomerizes to o-hydroxybenzonitrile, and no fragmentation is observed up to a temperature where the isomerization is almost complete (similar to 1040 K at 2 ms reaction time). The isomerization experiments in this investigation covered the temperature range 900-1040 K. The lack of fragmentation is in complete contrast to the thermal behavior of isoxazole, where no isomerization was observed and the main decomposition products over the same temperature range were carbon monoxide and acetonitrile. In a series of experiments covering the temperature range 1190-1350 K, a plethora of fragmentation products appear in the post shock samples of 1,2-benzisoxazole. The product distribution is exactly the same regardless of whether the starting material is 1,2-benzisoxazole or o-hydroxybenzonitrile, indicating that over this temperature range the 1,2-benzisoxazole has completely isomerized to o-hydroxybenzonitrile prior to fragmentation. Two potential energy surfaces that lead to the isomerization were evaluated by quantum chemical calculations. One surface with one intermediate and two transition states has a high barrier and does not contribute to the process. The second surface is more complex. It has three intermediates and four transition states, but it has a lower overall barrier and yields the isomerization product o-hydroxybenzonitrile at a much higher rate. The unimolecular isomerization rate constants k(infinity) at a number of temperatures in the range of 900-1040 K were calculated from the potential energy surface using transition-state theory and then expressed in an Arrhenius form. The value obtained is k(first) = 4.15 x 10(14) exp(-51.7 x 10(3)/RT) s(-1), where R is expressed in units of cal/(K mol). The calculated value is somewhat higher than the one obtained from the experimental results. When it is expressed in terms of energy difference it corresponds of ca. 2 kcal/mol.

2,1-Benzisoxazoles have been selectively synthesized through reductive heterocyclization of 2-nitroacylarenes using Pt supported nanoparticles. The reaction involves a cascade process in which the first step is the reduction of the nitro group into hydroxylamine followed by heterocyclization through the nucleophilic attack of the hydroxylamine group to the carbonyl of the acyl group and further dehydration. The reaction was performed on Pt/C, Pt/TiO2, and Pt/MgO using hydrogen as the reducing agent under mild reaction conditions. The results showed that Pt/MgO was the most active and selective catalyst. The study of the influence of the crystal size of the metal on the activity and selectivity, combined with the reaction mechanism examined by in situ Fourier transform infrared spectroscopy of the adsorbed reactant, showed that the maximum activity and selectivity to the target compound can be achieved by controlling the architecture of metal particles and acid base properties of the support. The effect of temperature on selectivity, the stability of the Pt/MgO catalyst, and the scope of the reaction have been studied. Finally, reductive heterocyclization using different metals (Pd and Au) supported on MgO has also been performed.
