In this paper, we report the optimization of a series of novel, potential antipsychotic derivatives combining potent dopamine D-2, D-3 and serotonin 5-HT1A, 5-HT2A receptor affinities. Objectives: Zonisamide (ZnS) is a new generation antiepileptic agent used in the treatment of epilepsy patients with partial and generalized seizures. In this study, we aim to investigate the safety and efficacy of ZnS in the treatment of patients with refractory epilepsy who were being followed. Methods: Forty-five refractory epilepsy patients who received ZnS treatment were included in this study. Patients who received ZnS treatment for less than six months were excluded. Age, sex, types of seizures, examination findings, magnetic resonance imaging and electroencephalography findings, concurrent use of non-ZnS antiepileptic drugs, decrease in the seizure frequency and side effects of the drug were recorded. Results: Thirty-nine patients, whose mean age was 34.3 +/- 9.3 years, were evaluated. Complex partial seizures (CPS) and generalized tonic-clonic seizure (GTCS) were observed in 74.4% of the patients, whereas 10.3% had GTCS alone, 7.7% had CPS alone, 5.2% had GTCS and myoclonia and 2.6% of them had absence and myoclonia. In the follow-up, treatment was observed to be discontinued in 19 of the 39 patients due to drug side effects, or where there was an increase, or no change, in seizure frequency. Twenty patients responded to treatment. Seizure frequency was decreased by 25% in one patient; 50% in five patients, and 75% in seven patients. Three patients were seizure-free. Although there was no change in seizure frequency, seizure duration was shortened in four patients. Treatment-responsive patients were using ZnS at doses that ranged from 100 to 400 mg/day for 7 to 80 months. Conclusion: ZnS is a safe, tolerable and effective option for the additional treatment of refractory epilepsy patients at our center.

Objective: Delirium is a life-threatening neuropsychiatric syndrome characterised by disturbances in consciousness, attention, cognition and perception. Antipsychotics are considered the drugs of choice in managing the symptoms of delirium. Paliperidone is a benzisoxazole derivative and the principal active metabolite of risperidone. In this study, we aimed to evaluate the efficacy of paliperidone for the treatment of delirium. Methods: A prospective open-label study of paliperidone for delirium treatment was performed with 6-day follow-up. Fifteen patients who met Diagnostic and Statistical Manual of Mental disorders, Fourth Edition criteria for delirium and had a score of 13 on the Delirium Rating Scale were recruited. The starting dose was 3 mg once a day and the dose was adjusted depending on the status of delirium. Daily assessments of the severity of delirium were evaluated using Memorial Delirium Assessment Scale (MDAS). Results: The mean daily maintenance dose of paliperidone was 3.75 +/- 1.06. The MDAS scores before and after treatment (day 7) were 23.60 +/- 6.31 and 11.33 +/- 5.45 (t = 6.78, p < 0.001), respectively. The intensity of delirium showed a statistically significant reduction in MDAS scores from the first day of treatment. No serious adverse effects were observed, and none of the patients discontinued paliperidone because of adverse effects. Conclusions: This study shows that low-dose paliperidone is effective in reducing behavioural disturbances and symptoms in delirium and is well tolerated in delirious patients. This trial is an open-label study with a small sample size, and further controlled studies will be necessary. A series of isoindoline analogues with either an indazole (HMR 2934, HMR 2651) or benzisoxazole (HMR 2543) appendage were prepared for the proposed treatment of psychiatric disorders such as obsessive compulsive disorder and attention deficit disorder. A selective and sensitive liquid chromatographic method was developed for the determination of zonisamide in small volumes of plasma. Zonisamide and the internal standard methyl 4-hydroxybenzoate were extracted from 0.2 mL. of plasma with solid-phase extraction columns and eluted with methanol. Analysis of the extracts was performed on a Symmetry C-18 column with ultra-violet spectrophotometric detection. The calibration curve was linear over the concentration range of 0.05-5 mug mL(-1) in plasma. Recoveries were reasonable for routine analyses; the limit of quantification was 0.05 mug mL(-1) with a signal-to-noise ratio of 5. This method could be useful for the pharmacokinetic study of zonisamide in a limited volume of human plasma and for therapeutic drug monitoring.

Risperidone is a new benzisoxazole derivative displaying a very potent serotonin antagonism and a potent dopamine antagonism in pharmacological studies. These properties suggest the hypothesis that risperidone may exert antipsychotic effects and be superior to classic neuroleptics in its beneficial effects on negative and affective symptoms and its low extrapyramidal side-effect propensity. In an open pilot study 13 patients suffering from acute schizophrenic psychosis were treated with risperidone within an individually adapted dose range from 1 to 10 mg per day. A good antipsychotic efficacy could be demonstrated in 6 of the 8 patients who completed the trial. Risperidone was very well tolerated. The substance possesses a low EPS-inducing profile. Future research has to test the suggested advantage of risperidone over other neuroleptic drugs and its performance in the treatment of chronic schizophrenic patients.
