In addition, MS/MS measurements of fragment ions show characteristic differences that allow distinction among the isomers depending on the original arrangement of the atoms in the five-membered ring. Theoretical ab initio calculations have allowed to determine chemico-physical properties of different ions and to propose a rationalization of the decomposition pathways followed by the two benz(is)oxazole isomers. DETERMINATION OF ZONISAMIDE (3-SULPHAMOYLMETHYL-1,2-BENZISOXAZOLE) IN PLASMA AT THERAPEUTIC CONCENTRATIONS BY HIGH-PERFORMANCE LIQUID-CHROMATOGRAPHY

Zonisamide: A Review of Its Use in the Management of Adults with Partial Seizures

Oral zonisamide (Zonegran (R)) is a benzisoxazole derivative chemically unrelated to other antiepileptic agents. Zonisamide was generally well tolerated in adults with partial seizures participating in these studies, with the majority of adverse events being mild or moderate in severity. Thus, oral zonisamide as monotherapy or adjunctive therapy to other AEDs provides a useful option in the treatment of patients with partial seizures.

Death following an intramuscular injection of paliperidone: A case report

Paliperidone (9-hydroxyrisperidone), the primary active metabolite of risperidone, is a benzisoxazole derivative of the second-generation antipsychotics. Paliperidone patmitate or PP (the patmitate ester of paliperidone) is available as a once-daily oral tablet, a once-monthly injection or PP1M, and a 3-monthly injection or PP3M (marketed under the name of Trevicta (R) in the European Union). Despite the recent availability of PP3M, several cases of intoxication have been reported worldwide, including lethal cases. However, in Europe, no case of fatal poising has been reported to date. A 33-year-old man was found dead in his room at his parents’ home. His only medical background was a history of schizophrenia. The external examination was without any particularity except a runny nose of brownish liquid with an alcoholic smell. Given the context and the tack of evidence of a third party intervention, an autopsy was not considered necessary. Toxicological analysis of right femoral blood highlighted the presence of paliperidone (240 mu g/ L) only. The risk associated with the use of PP3M formulation is still imperfectly evaluated and several cases of intoxication have been reported worldwide, including fatal cases. In this case, paliperidone poisoning appears to be the highly likely cause of death. In fact, the blood concentration of paliperidone is high, twice the toxicity threshold used in therapeutic monitoring, and no other substances have been identified. This case highlights: the necessity to always evaluate the benefit-risk balance when prescribing this sustained-release form of paliperidone and the difficulties in interpreting paliperidone concentration in forensic cases. Genotyping as a Key Element of Sample Size Optimization in Bioequivalence of Risperidone Tablets

Risperidone is a derivative of benzisoxazole and is widely used for schizophrenia and other psychiatric illnesses in both adults and children. Previous studies have confirmed that it is a highly variable drug (within-subject variability >= 30%). To reduce the large sample size required for bioequivalence researches on highly variable drugs, a role for genotyping in the design of the bioequivalence study was employed. A randomized, open-label, two-period crossover study was adopted: 20 subjects with specific genotypes carrying cytochrome P450 (CYP) 2D6*10 were randomized to two groups to receive a single oral dose of trial formulation or reference formulation with a 2-week washout period. Blood concentrations of risperidone (parent drug) and 9-hydroxy risperidone (active metabolite) were measured by high-performance liquid chromatography-tandem mass spectrometry. Eighteen out of the 20 subjects completed the study (two did not finish the test in the second period). The pharmacokinetic parameters of AUC(last), AUC(a) and C (max) for the 18 subjects after a single oral dose of the trial or reference preparation were 216.1 +/- 88.7 and 220.5 +/- 96.8 ng center dot h/mL; 221.6 +/- 93.1 and 226.4 +/- 103.5 ng center dot h/mL; 36.7 +/- 10.3 and 36.0 +/- 10.2 ng/mL, respectively. The CVw of risperidone in natural logarithm-transformed C (max) was 22.4 and 25.38% for 9-hydroxy risperidone. The test formulation met the Food and Drug Administration guidelines and regulation criteria for bioequivalence. By controlling the genotype, it could actually help reduce the CVw, which may be a feasible method to decrease the sample size for the bioequivalence study of highly variable drugs.

A practical procedure for synthesis of a new uricosuric agent, 5-chloro-7,8-dihydro-3-phenylfuro[2,3-g]-1,2-benzisoxazole-7-carboxylic acid (1, AA-193) is described, which starts from 2,5-dichlorophenol (3b) and involves 5-chloro-6-hydroxy-3-phenyl-1,2-benzisoxazole (2) as the key intermediate. Successive treatment of 3b with benzoyl chloride-aluminum chloride (AlCl3) and hot ethanolic sodium hydroxide gives 4-benzoyl-2,5-dichlorophenol (8, 61%), which is oximated with hydroxylamine hydrochloride and then transformed into the benzisoxazole 2 (88%) with potassium hydroxide in N,N-dimethylformamide (DMF) (method C). The reaction of 2 with aqueous formaldehyde and dimethylamine affords the Mannich base Ila (97%), which is treated with a sulfonium ylide 12, 14 or 15 followed by heating with sodium hydroxide (NaOH) in ethanol (EtOH) to give 1 in high yield (method E).
